The New Template
I think it should be like this:
The Uzi Submachine gun has appeared in the following:'
Film
The new template as created by PhoenixEnt

And if it's a televison show it should be "In the Television Show"
And if it's a Video Game it should be "In the Video Game"
And the same applies to the "Date of Film/Televison Show/ Video Game".

- Actually, we could just say "Title", and that would work just as well. Other than that, I think the columns as MPM has named them work best.
