As the wheel turns William decides who lives or dies by either enduring pain himself to save two of his choice or not doing anything for the other four and when the person stops in front of the shotgun if William chooses to not do anything, the mechanism the shotgun is rigged to, aimed at them cycles the action and fires at them; killing them.
