This shotgun is fitted with an extended magazine tube, pistol grip, door breacher muzzle brake, and collapsible KNOXX SpecOps NRS stock. A short barreled version was used by Chin Ho at the end of "Na Pilikua Nui" (S6E06) as its first appearance.

It is equipped with a Noveske KX3 flash hider, Trijicon SRS red dot, a Magpul STR stock, Magpul VFG, Magpul XTM rail covers and hand stop, and is occasionally loaded with either a Surefire 60-round magazine or a PMAG 40. Gabriel appears to use it to assassinate Honolulu Yakuza oyabun Goro Shiyoma near the end of "Ka Makua Kaa Kaua" (S6E10).
McGarrett shoulders the Mk 18 Mod 1 during a hostage rescue mission near the end of "Mai ho`oni i ka wai lana mâlie" (S6E01).
